Our Mission Statement:
We are dedicated to improving access to health care by assisting individuals with costs related to medical and basic needs.
Many individuals struggle with medication costs, finding transportation to and from medical appointments, food insecurity, housing costs, and affording durable medical equipment, such as walkers and oxygen tanks. All of this and more can significantly impact the health and wellness of patients in our community. Our mission is to provide individuals with patient-centered health services and treat them with excellence in quality of care, service, and access to assistance. Our goal is to care for the whole person by addressing both health and social needs. Colorado Patient Care Fund aims to remove barriers being faced in order to receive high quality health care.
